Bariatric Evaluation
Initial visit: $215.00

One hour follow up visit: $110.00

Half an hour follow up visit: $85.00

Follow up packages:
5 - 30 minute appointment $200.00*
5 - 15 minute appointment. $100.00*
* packages must be used within 6 weeks of first visit
